New Patient Information

Bring Any Medical Information or Reports Relating to Your Care With You

  • You will be asked for personal information about your medical and family history. We encourage you to fill out the patient history form. You can download it at the bottom of this page.
  • Make a list of all your medications and bring it with you to your appointment.
  • Make a list of your allergies and bring it with you to your appointment.
  • We will ask you for your preferred pharmacy information. We will enter your pharmacy name, address, and phone number in your electronic health record. This will enable us to send prescriptions for you directly to your pharmacy.
  • Bring your insurance card, personal identification card, and a means of payment if you have a co-pay. We accept Cash, Check, Visa, Mastercard, Discover, and American Express.

Late Arrival Policy

  • If a patient is more than 15 minutes late for an appointment, the appointment WILL be rescheduled to not inconvenience our other patients' appointment times.
  • New patients are encouraged to print the forms and fill them out prior to arriving at the office. If you choose to complete the necessary paperwork in the office, you must arrive 30 minutes early. If the paperwork is done late we may need to see other patients who arrive on time for their appointment.

Before your appointment

  • Check with your insurance company to see if they require a referral. If your plans require to have a referral please obtain it prior to your appointment or we will have to reschedule.
  • We accept most major insurance plans. Coverage requirements and limitations vary from plan to plan and from time to time. We advise you to contact your insurance company prior to each appointment to inquire if our practice or your physician is a participating provider. We have provided a list of insurances companies we are contracted with. Understanding your policy, co-pays, and deductibles is essential for determining your out-of-pocket expense.

Request Refills of Medications

  • Refills on medications should only be done by calling your pharmacy at least 1 week prior to your last refill and having them send us a refill request.
  • For Class II Controlled Substances, a special hard-copy prescription form is required by Florida State Law. Refills on these meds will need to be handled in person with your Dr. Goshen during your appointment. Prescriptions that are lost, stolen, or destroyed will not be replaced.

After-Hours Assistance

  • Routine medication refills will not be done after hours.

  • Refills of controlled substances will not be done after hours. No exceptions.

  • Antibiotics will not be called out without an office visit. See above information.

  • If you are having a life-threatening medical emergency, you need to call 911 or go to the nearest emergency medical facility.

Get Results of Labs or Other Tests

All lab tests must be ordered by Dr. Goshen, thus you will need an appointment with your provider to get any lab test ordered and you will be required to make a follow-up appointment to discuss results. Under NO circumstances will results be given over the phone.
Lab companies also are responsible for billing your insurance company for lab tests that are done through our office. Check your bill to see if it is from a lab company, such as LabCorp or Quest Diagnostics, and if so you should contact them directly at the phone number listed on the bill.
